If your fixture or kit has failed and falls within warranty, please complete this form and send to your rep or factory contact. Please refer to TechBrite’s statement of warranty to check if your product falls within the warranty period.
FOR A NO CHARGE REPLACEMENT PLEASE CHOOSE ONE OF THE FOLLOWING OPTIONS
Option 1: Ship the defective back to TechBrite prior to receiving the replacement.
Option 2: Provide a credit card number to ensure the return of defective. Replacement will be sent immediately. The credit card will be charged if defective is not returned within 30 days of replacement shipment or if the return is found to be defective due to user error.
Option 3: Issue a PO for the replacement with cost. Upon return of the defective the invoice will be credited.
